If you believe that any of the information we maintain about you is incorrect or incomplete, you have the right to request an amendment. Contact us to initiate this process.

You have the right to specify how you would like Private Medical to contact you should we need to do so. To update your preferred communication method, contact our support team at 415-830-3090 for assistance.

You may request a list of disclosures of your personal information, including those shared for legal or operational purposes. To request this information, contact our Privacy Office.

You may grant authorization for someone else to access or manage your medical records on your behalf. To do so, please complete an authorization form available upon request.

If you believe your privacy rights have been violated, you have the right to file a complaint with Private Medical or with the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.

If a patient of Private Medical has passed away, please notify us so that we can properly update our records and assist with any necessary arrangements.

Certain data-sharing initiatives may be used to improve healthcare services. You have the right to opt out of participation in such initiatives. Contact us if you wish to do so.
